Боевые стрессовые расстройства у военнослужащих, включая посттравматическое стрессовое расстройство (ПТСР), возникающие вследствие воздействия на психику стресс-факторов боевой обстановки, приводят к социальной дезадаптации и снижению боеспособности. Своевременная диагностика и последующая профилактика проявлений различных форм стресс-ассоциированных психических расстройств позволяют избежать их хронического характера и сопутствующей психосоматической патологии.</p><p>Цель – выявить биопсихосоциальные маркеры ПТСР у комбатантов.</p></sec><sec><title>Методология</title><p>Методология. Проведены клиническое психопатологическое обследование, биохимические и химико-токсикологические исследования для выявления метаболитов психоактивных веществ. Обследованы 85 военнослужащих-мужчин Росгвардии после пребывания в зоне боевых действий в 2022–2023 гг. сроком до 90 сут, в том числе, 1-я группа (n = 38), проходившие амбулаторное лечение с диагнозом ПТСР (F43.1 по МКБ-10), средний возраст – (28,4 ± 2,2) года, стаж службы – (2,9 ± 1,4) года; 2-я группа (n = 47), не имевшие психических расстройств, средний возраст – (29,2 ± 2,1) года, стаж службы – (3,1 ± 1,2) года.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлены значимо высокие показатели тестов в 1-й группе по риску ПТСР, уровню тревоги и депрессии. У них выявлено наличие положительных достоверных взаимосвязей между ПТСР и биохимическими показателями, уровнями этилглюкуронида, тестостерона и вечернего кортизола.</p></sec><sec><title>Заключение</title><p>Заключение. С учетом особенностей социально-политической ситуации, специфики масштаба и интенсивности военных операций необходимо изменение подходов к проведению обследований комбатантов с разработкой многоосевых – биопсихосоциальных маркеров стресса, включающих как осуществление линико-психологического скрининга с использованием методик, регламентированных Минздравом России и МВД России, так и данных химико-токсикологических и биохимических исследований. На фоне тенденции роста значений по шкалам опросников TSQ и HADS дополнительными маркерами развития ПТСР у участников боевых действий могут служить повышенные уровни тестостерона, вечернего кортизола, карбогидрат-дефицитного трансферрина и наличие этилглюкуронила и бензодиазепинов в биологическом материале.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Military personnel combat stress disorders, including post-traumatic stress disorder (PTSD), arising from the impact of the stress combat situation factors on the psyche, lead to social maladaptation and decreased combat capacity. Timely diagnosis and further prevention of various stress-associated mental disorders helps to avoid progression into a chronic disease and concomitant psychosomatic pathologies.</p><p>The objective is to identify PTSD biopsychosocial markers in combatants.</p></sec><sec><title>Methods</title><p>Methods. Clinical psychopathological examination, biochemical and chemical toxicity studies (CTI) were conducted to identify metabolites of psychoactive substances. 85 male officers of the Russian Guard were examined following up to 90 day’s deployment in the combat zone in 2022–2023; those were split into Group I (38 individuals who underwent outpatient treatment for PTSD (F43.1), average age (28.4 ± 2.2) years and service experience (2.9 ± 1.4) years) and Group II (47 individuals with no diagnosed mental disorders, average age (29.2 ± 2.1) years and service experience (3.1 ± 1.2) years).</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The two groups revealed significant differences for all test parameters, i.e. PTSD risk, anxiety and depression levels. The group that included combatants seeking psychiatric aid demonstrated remarkably higher levels, revealing positive reliable correlations between PTSD and biochemical parameters, ethyl glucuronide, testosterone and midnight cortisol.</p></sec><sec><title>Conclusion</title><p>Conclusion. Taking into account specific social and political settings, as well as the scale and intensity of military action, further improvement is demanded in the examination of combatants using multiaxial biopsychosocial framework of markers, including both clinical and psychological screening, as well as chemical, toxicological and biochemical studies, as prescribed by the regulation of the Ministry of Health of Russia and the Ministry of Internal Affairs of Russia, is necessary to change.
